Travel During COVID-19—Passenger attestation to enter the U.S.

Do NOT travel if…


Do not travel until a full 5 days after your last close contact with the person with COVID-19.[2] It is best to avoid travel for a full 10 days after your last exposure.
If you must travel during days 6 through 10 after your last exposure:
Get tested at least 5 days after your last close contact.

Passenger attestation to enter the U.S.

The U.S. government requires all passengers flying to the U.S. - 2 years and older - to complete the Center for Disease Control and Prevention (CDC) attestation form confirming they meet entry requirements. If you're a non-U.S. citizen you must also have proof of vaccination against COVID-19 to fly, unless you qualify for an exception.
